Parent Consent

We need your permission in order to include your child in the Active for Life Year 5 study. The type of consent that we are using in this study is ‘opt out’ consent. This means, you only need to contact us if you DO NOT want your child to be involved in the research. This method has been approved by the University of Bristol, Faculty of Health Sciences Ethics Board.

When your child’s school joined the study, either at the end of Year 4 or the start of Year 5, you will have been sent an information sheet (PDF, 154kB) (pdf, 155Kb)and an opt-out consent form (PDF, 73kB) (pdf, 73.9Kb) in the post, directly to your home. You only needed to return the form if you DID NOT want your child to take part in any aspect of the study. If you were happy for your child to take part, you didn’t need to return the form.

We collect data from your child 3 times:

1. End of Year 4 or start of Year 5

2. End of Year 5

3. End of Year 6

If you returned a completed opt out form, then your child will not be included in the measurements at any of the 3 time points.

If you did not return the form as you were happy for your child to participate, they will be offered the chance to join in the measurements on all 3 occasions. Each time, we will ask each child individually whether they would like to participate and which measurements they would like to do. They are able to opt out of any measurements they do not wish to do on the day and this is fine.
