Avoiding primary care sensitive hospital admissions

1. What proportion of unplanned hospital admissions is attributable to existing definitions of primary care sensitive admissions, including ACSC?  Are there other admissions, particularly common admissions or those that incur heavy resource use, which may be primary care sensitive?

2. Can primary care clinical record data be used to identify patients at high risk of unplanned hospital admission?
