The Learning Disabilities Mortality Review (LeDeR) Programme team at the University of Bristol has developed a national repository* of case reports pertaining to people with learning disabilities.

This collection of reviews holds summaries of Safeguarding Adult Reviews, Serious Case Reviews, and Serious Incident Reports. 

We have worked with Local Safeguarding Adults Boards and NHS Trusts to collect Safeguarding Adults Reviews (formally serious case reviews) and Serious Incident Reports to create a national repository pertaining to people with learning disabilities. Also incorporating Ombudsman Reports, the Repository is formed of examples of deaths or ‘near misses’ (serious incidents), with key learning points and recommendations for each case.

These have been thematically analysed (see Repository Analysis below) to draw out common learning points about the deaths of people with learning disabilities which can be shared nationally.

This repository includes links to the original reports, where they are available online. The names we have used, whether they have been anonymised or changed to protect identities, are copied from the original publicly-available reviews so that our summaries can be easily identified and linked with the originals.

*This repository is separate to the rest of the programme. It does not contain any reviews which have been carried out as part of the LeDeR programme.
